I understand from CEGA tha= Myla has now been admitted to hospital for the surgery, estimated at 100,0=0.00 PHP 
(approximately GBP 150.00) and CEGA are sending a guarantee of payment to the hospital for the procedure. I 
und=rstand that Myla is expected to require 1 months rest period to recover pos= operatively. I therefore await further 
updates on Myla's recovery f=rm CEGA in due course.

In the meantime, I would b= most grateful if you could please arrange for the Insured to complete and r=turn the 
attached Sturge medical claim form in due course, along with any additional supporting information / documenta=ion 
that the Insured may have and including any medical expenses incurred. 
As I mentioned in my email=of 29th April 2016, the Insured are entitled to claim Temporary T=tal Disablement benefit 
(T.T.D or salary benefit) in respect of the tie that Myla is medically certified as un=it to work, subject to receipt of 
supporting medical certification from a t=eating doctor confirming the period of disablement in question and less 
the=policy excess of the first 7 days of disablement. With this in mind, if the Insured would like to submi= a claim in this 
respect, I look forward to receive the supporting me=ical certification in due course along with formal confirmation of 
Myla =99s salary as of August 2015, (which is when I understand that she received her diagnosis) by way of a copy of her 
wage s=ip or relevant page of her contract of employment.
