Ortiz of Medford, Ore. passed away November 20, 2015 at RRMC surrounded by family. She was a devoted wife, mother, grandmother, and daughter. Hazel was born April 2, 1931 in Medford to Frank and Mary (Lewis) Silva. Her parents homesteaded property on the top of Roxy Ann which is still in the family. Hazel married George A. Ortiz June 12, 1946 in Medford. They were married 64 years before her loving husband passed away. She retired from Bear Creek Corporation with 20 years of service. Hazel and George had a summer place in Brookings, Ore. where they spent all summers. Hazel was a long-time member of the First United Methodist Church, Medford. Hazel is survived by her daughters, Maria C. McLaughlin (Dick), Sandi Buttolph (Steve), and Diana Nokes (Russ); son, George A. Ortiz Jr. (Candy); grandchildren, Eric Smith, Chris Ortiz, Mindy Shaw, Brandy Ortiz-Moore, Kari Potter, Stephen Buttolph, Shannon Dintleman, Eric McLaughlin, Scott McLaughlin, and Jennifer Harvey; 19 great-grandchildren and one great-great-grandchild. She was preceeded in death by her husband, George A. Ortiz; parents, Frank and Mary Silva; brothers, Joseph Silva, Frank Silva, and Leonard Silva, and sister Lena Silva. A memorial service will be held Saturday, December 5, 2015 at 11:00 a.m. at the First United Methodist Church 607 W.
