Oregon State Police reported 63-year-old Jacqueline Ihara-Stafford was found dead at the scene by responding crews. Another Cave Junction woman Carolyn Tillett, 61, who was a passenger in Ihara-Stafford's car, was taken to a local hospital with injuries considered "non-life threatening," according to an OSP news release. The crash was reported at 2:40 p.m. at milepost 13 on the highway. A preliminary investigation shows Ihara-Stafford was driving a 2000 Mazda sedan west on Highway 199 when the car drifted off the road and rolled over. Ihara-Stafford was not wearing a seatbelt and was ejected. OSP, American Medical Response, Illinois Valley Fire & Rescue, and the Oregon Department of Transportation responded to the scene. The highway was closed intermittently for several hours during the initial investigation.
