Moderna and Pfizer boost. There is a tension between her/CDC
and Peter Marks and Larry Horowitz (FDA) regarding whether or not the younger children
should all get the bivalent boost or have them finish their primary regimens with the ancestral
strain vaccine. Peter and Larry had both called me up previously (couple of nights ago) making it
look like the CDC was holding back on wanting to give the most updated boost to the children.
Yet, when Rochelle explained this to me this morning it became clear that it is a much more
complicated situation than Peter and Larry described to me by phone a couple of nights ago.
Ultimately, in January all children of all ages will get a unified vaccine either for the primary
series or the boost and that will be the bivalent BA.4/5. Rochelle explained that to insert that
bivalent boost into the already complicated regimen for the small fraction of children who have
started on their primary series would create an enormous amount of confusion for pediatricians. I
tend to side now with Rochelle even though when Peter and Larry explained that in the
simplified form it seemed that they were making sense. However, according to Rochelle's
explanation I believe she is correct. We will all sort this out on a zoom call with HHS tomorrow
(Sunday) evening.

Had a Sunday evening Zoom call regard the situation of boosts for children 5-6 years and
younger (see above). It involved: me, Rochelle, Robbie Goldstein, Peter Marks, Rob Califf,
Dawn O’Connell, Vivek Murthy, Larry Tabak, among others. We all agreed that this was a
close call and both sides have a good point. The important issues is that this involved only
about 5% of the eligible children since the other 95% have not yet received their primary
vaccination and those are truly the vulnerable ones if we get a winter surge. At the end of
January, 2023, the VRPAC will meet to recommend that going forward all vaccinations (primary
series and boosters) will be the BA.4/5 bivalent and so this will be a moot point. The question
still remains unsettled as to give the children the best boost now even though it logistically will
be difficult for the pediatricians since they will have multiple (11) different products in their
offices from Moderna + Pfizer) or wait until we have a uniform boost for everyone at the end of
January. No firm decision was made. FDA will try to push the date for VRPAC closer to close
the gap. More to come

Presentation by the Shionogi (Takeki Uehara) Phase 3 data. The drug is an anti-viral
similar to Paxlovid. The data were not particularly impressive since the study was done on
people with very few symptoms and all the drug did was diminished by 24 to at the most 48
hours the duration of the symptoms. The study was not designed to determine whether or not the
drug prevents the need for hospitalization than people with a high risk of severe outcome. All in

all I was not impressed with the data because they were looking at the wrong population of
patients.

I addressed the UK Parliament today on lessons learned from the COVID pandemic. It was
followed by a very productive and insightful question period from the members of
parliament
Lawsuit by officials from Louisiana and Missouri have ruled that several government
officials including me, Jen Psaki, Vivek Murthy among others must undergo a deposition
since we are charged with trying to influence his social media to suppress the free speech of
a number of people including those involved with the Great Barrington Declaration. It is a
colossal waste of time since there is absolutely nothing there and the prep for the deposition
would be about 8 hours over three days and the deposition itself which I will do tomorrow will
be 7 full hours on the clock. This is total harassment. We tried to get the deposition stopped, but
a Louisiana judge ordered that we have to do it. Just yesterday another judge overturned the first
judge and the beck murthy does not have to be deposed, but I still have to be.
***Extraordinary press briefing in the White House Press Room today. Today was an
historic day for me in that the White House comms team led by Karine Jean-Pierre and
Kevin Munoz arranged for me to be part of the press briefing from the White House
briefing room podium. it turned out to be a celebration of my nearly 40 years getting
briefings at the White House. The moving part about it was that both Karine and Kevin
were brought to tears when they said goodbye to me after the briefing. In addition, all of
the press room personnel wanted selfies with me before I left. In addition, the briefing
room was packed with media people who are there to hear the latest update on COVID but
they directed most of their questions to me related to my retirement, my legacy, and other
things that had direct relation to me. It was a very moving and historic time for me.

I had my 7 hour (on the clock) deposition by lawyers for the Missouri Vs Biden plaintiffs. It
showed up as spectators as the plaintiff’s lawyer John Sauer questioned me for seven
hours. I had been well prepared for the previous few days by the DoJ lawyers including Adam
Kirschner and Josh Gardner as well as Amanda Chuzi Anant Humar, Brian Netter and HHS
council Chris Hammond. Although this was supposed to be on the topic of the Biden
administration including me interfering with free speech by influencing the social media
platforms to not allow opinions differing from our own to be put on social media, very many of
the questions were directed at things unrelated to that including the origins of COVID, the
Wuhan grant, Peter Daszak, my position on masking, and a variety of other issues that the
Congress has been concerned about. I must say I did extraordinarily well and the DOJ lawyers
said that that was one of the best performances and the deposition that they have ever seen.

I am receiving so many wonderful warm and affectionate notes from people on the occasion
of my stepping down. One among many typical notes came from Beth Cameron of the
NSC. Beth texted the following note to me late last night associated with the prior note
wishing me a happy Thanksgiving: “Be well. And thank you so very much. I very much hope
to continue to learn from you in your future roles, and I also hope you are able to get a little bit
of time to rest (though that may not be in your DNA!) All the very best. I stand in awe of not
just what you have done but how you have done it and with the humanity with which you have
done it.”

I received a call yesterday from Dori Solcido and a similar call last night from Ashish Jha
telling me that Ron Klain was upset mostly at Ashish for indicating in interviews that we
could expect a mild winter. Ron was concerned that we should not be in the business of
predicting lack of severity of COVID for this winter because if we have a bad winter with
COVID then confidence in the government would diminish. Ron has a point there, but I'm not
sure exactly what Ashish said. According to Dori, the New York Times made it look like Ashish
was saying that we don't need to worry very much over the coming winter. I know as a fact that
that is not what he said but he is a bit in the doghouse now with Ron because of that. This just

Met with in 31/7A-18 Ambassador Pamela Hamamoto who is US Negotiator for the
Pandemic Accord. We (Cliff, Gray Handley and I) brought up some of the landmines that
might arise in such an accord regarding conditions placed on receiving reagents. Also, we
brought up the recurring issue of the problem with WHO feeling that they should be directing the
research associated with outbreaks – long history here (see Cliff’s experience with Anna Maria
Restropo.

I had an enjoyable “fireside chat” at the White House Jackson St facility in which I had
lunch with the White House Fellows. I enjoy this very much and have done this several times
over the past few years. They are an outstanding group of about a dozen individuals from various
walks of life many of which have served in the military including in the special forces; practicing
physicians; lawyers, among others. I enjoy speaking with them because they are the cream of the
crop and give me hope for our nation particularly in the context of the incredible divisiveness
and disregard for the truth and integrity that we are seeing now in our nation.

Disaster!!! Chris felt ill last night and decided to sleep in Jenny’s room. She felt worse
this AM and tested positive for COVID. I have started her on Paxlovid. I'm sure she will do
fine, but this has completely destroyed one of the most intensive social commitments that we
have over the next few days. I have decided that even though I have now tested negative multiple

times that I really cannot go to the state dinner tomorrow (Dec. 1) at the White House since it
would be unconscionable for me to be secreting virus while being asymptomatic and antigen
negative and endanger the president of the United states. In addition, I will have to modify my
visit tonight to the home of the Italian ambassador (Mariangela Zappia) who invited people from
all over the country including California to come to the ambassador’s home for a celebratory
dinner in my honor. I went there tonight and did a fireside chat with a mask on with a well
known Italian author Antonio Monda, and then left before the dinner since I did not want to take
my mask off. I will also have to Mr planned dinner with Bob Seder and Jeff Zients and families
on Saturday.

I am really being somewhat embarrassed by the amount of honors that are be given to me
and kudos my way. For example, today we went to the Hay-Adams hotel for a ceremony
commemorating PEPFAR and world aids day. The ceremony involved multiple speeches
including a speech by secretary of state Tony Blinken in which he effusely praised me for
my role as an architect of pepfar as well as for decades of my contribution to HIV aids
research and research in general. In addition, we had a fireside chat led by Sandy
Thurman between John Kkengasong and myself. The entire ceremony at the hay Adams
included multiple crazy by several people on me. As I said it was somewhat embarrassing
since I do not like that kind of attention.

Wonderful afternoon at the White House and evening at the Kennedy Center. Actor
George Clooney, singer-songwriter Amy Grant, singer Gladys Knight, composer Tania
León and rock group U2 were celebrated as the Kenney Center Awards. I was the guest of
secretary Becerra at a White House reception with the President that occurred at 3:30 PM before
we all motorcaded to the Kennedy Center for the ceremonies of the awards. It was a star-studded
evening and I had pleasant conversations with people some of whom I had met before such as
Julia Roberts, Sean Penn, Matt Damon, Mike Ferrone, among many others. It was a spectacular
event mixing entertainment with honoring the honorees. The situation was somewhat unusual as
I was completely swamped with people yelling my name and applauding it as I left the vehicle in
front of the Kennedy Center and walked that long red carpet walk to the theater. There must have
been hundreds of people lined up behind a rope down area and I was amazed how this
recognition of me has reached such a height. I am certainly not getting a big head about it but it
is rather weird and a little bit unnerving. Joked with Hunter Biden as we shook and hands and
hugged. He said if someone snapped a photo and put it on line, the internet “would explode”
given we are both being attacked by the far right GOP

A very touching tribute and “good-bye” to me by the AAI Council. I regularly for 38 years
on an annual basis address the council here in Bethesda, they gave me a very warm last meeting
with a prolonged standing ovation as I walked into the room.
Terrific evening at Jeff Zients’s home. He had a reception on the occasion of making
available online the tapes of conversations between Richard Stengel (Editor of Time Magazine)
and Nelson Mandela. Mandela attended Jeff and Mary’s wedding as did Richard. Had nice
conversation with Merrick Garland and I encouraged him that we have faith in his judgment
and integrity to do the right thing regarding the issues with Trump. Also, finally saw in person
Katy Kay who has interviewed me multiple times virtually over the years for the BBC

We had our 9th annual meeting today with Bill Gates in the context of the NIH-Gates
Foundation collaboration on multiple projects. In the course of the meeting they showed to
my surprise film clips of Bill Clinton, Tedros, and Slim and Quarraisha Karim thanking me
for my many years of service. I should comment here on the really close relationship that I've
developed over the years with Bill Gates and the mutual respect we have for each other. We
also joined by the common issue of the conspiratorial theorists out there who think that Bill and I
are doing strange things like putting chips in the vaccines that we're trying to get them to receive.
It's the source of fun jokes between Bill and me.
Senator Richard Burr (R-NC) called me today to wish me well and to tell me how much he
has enjoyed over the years working with me. He congratulated me for my “extraordinary
service to our nation over decades”.
Four astronauts visited the Clinical Center today and the Children’s Inn. The said that the one
“must thing” that they wanted was to meet me and so I went over to the Medical Board meeting
on the 4th Floor and met with and took photos with them: Thomas H. Marshburn, Kayla Barron,
Matthias Maurer, Mark T. Vande Hei
Just met with the German Minister of Health Karl Lauterbach who has become somewhat
of a friend in that every time he is in DC he wants to come and see me. He values my advice
and says that I am a “rock star” in Germany. Also, at the meeting was German Ambassador to
USA Emily Haber.

PSA for AARP encouraging elderly people to get vaccinated. Jill was very kind in the
introduction she announced that this was the last time that I would be on this stage with her and
she will miss me greatly. When she left the event she gave me a big hug and said she loved me

I went to Boston to receive the Edward M Kennedy award for Inspired Leadership at the
Edward M Kennedy institute for the United states Senate. It was a really nice affair and I was
showered with affection and admiration. A person who is a Broadway actor/singer (Alan H.
Green) Sing a beautiful song from Les Miserables and dedicated to me and as he was mentioning
the dedication he emotionally choked us. Very moving. I was introduced and had a Fireside

Momentous Day!!! Today we had the ceremony for my “stepping down” at NIH. I chose
a simple ceremony with 3 minute speeches by Larry Tabak, Francis Collins, John Gallin,
Cliff Lane and Christine. This was followed by a 15 minute Q and A with John Burklow
and a concluding speech by me. I thought that I would get through the entire speech
without choking up but when I got to the point about Christine, I choked up for about 15
seconds. The ceremony was exactly like I wanted it – no bells and whistles.

It is clear (I have not been reporting it every day) that the far, extreme right and Qanon
literally hate me and De Santis is starting to play into this with his growin campaign for
presidency. From the New York Times: Although “vaccine paranoia and Fauci hatred are still
niche positions in the electorate at large,” if anyone “imagines that replacing Trump with DeSantis as
the G.O.P.’s leader would signal a party on its way to becoming sane again is in for a rude shock.”
