Household transmission of COVID19 in Costa Rica

We have designed a questionnaire and data entry system. The questionnaire is being piloted and recruitment strategies are being discussed. A rapid reporting system is being established to refer confirmed cases from the Social Security to study personnel for potential enrollment in the prospective study. Recovered cases will be contacted and visited in their homes for potential recruitment into the prospective study. At the time of the home visit, household contacts will be enumerated and identified for potential recruitment into the household transmission study. Households eligible for selection will be those with at least one other household member besides the index case. This approach has been used previously in influenza household transmission studies in Central America. After administration of the informed consent, epidemiologic and clinical data will be obtained by interview or retrieved from medical records and blood samples will be collected. For minors 12-18 years old, informed assent will be obtained and at least one parent will sign informed consent. For children under 12, only one parent will sign the informed consent documents but verbal assent will be obtained when possible. We will obtain several types of information from household contacts. For each individual we will obtain information including 1) intensity and duration of exposure to index cases; 2) Use of preventive measures such as masks, 3) history of potential exposures outside of the home; 4) duration of quarantine as applicable; 4) history of underlying medical conditions that have been associated with disease development, 5) history of signs and symptoms compatible with COVID19, since onset in the index case. For each household, we will obtain information related to crowding and socioeconomic status.
